Schedule your first visit
Please schedule your initial visit within two weeks of registering. Your appointment itself may take place at a later date, depending on availability.
To keep my patient panel current, registrations without a scheduled initial visit within two weeks may be marked inactive. You can always reach out to reactivate your registration when you're ready to get started.
